Scantron Instructions for Students

Use only #2 pencil
Name must be bubbled in with one space only between first and last name. Last name only is acceptable.
ID field must be bubbled in with nine digits only. Nine digits are required. Do not put anything in the tenth bubble. Student may use any unique combination of nine digits.
Mark A in Test Form field unless instructor designates B, C, or D.
"Subj Score" field must be bubbled in with 000 -> Zeros only.
Do not make random marks anywhere on exam sheet.
Do not “X” out unwanted answers. Erase all unwanted answers.
